Healthy sexual practices. Sex in infertile women is mostly unplanned, without concern for fertility, and without any concern for conception. In addition, polycystic ovary syndrome, one of the most common endocrine disorders in women, can reduce fertility and sexual satisfaction. Women with this syndrome have irregular and unpredictable menstrual cycles, and ovulation occurs rarely. Because of the reduced fertility and the irregular ovulation, the sexual activity of infertile women is also reduced (27). The lack of acceptance and sexual knowledge about contraception and sex may make infertile women less prone to have sexual intercourse. In the case of menopause, the rate of sexual activity decreases. Finally, sexual activity is lower in older women. Many older women may not want to have sex because of their advanced age and the fact that they have children already or because they are too tired. Furthermore, older women have impaired sexual response and functionality compared to younger women. Therefore, it is usually less likely for older women to experience sexual satisfaction and sexual pleasure than for women at the peak of their sexual activities (28-30). In addition, older women experience more difficulty in the ejaculatory phase than younger women due to vaginal atrophy and the lack of lubrication, thus leading to a decrease in sexual satisfaction and pleasure (31). To help women become more confident in the area of sexuality, they should be educated about the aging process, the changes that occur during menopause, and the importance of sexuality. More importantly, they need to be encouraged to discuss their sexual issues with physicians and other health care providers (27).
In the case of anovulation, one must consider the possibility of ovulation inhibition. In the first place, sexual activity may interrupt the normal course of ovulation and menstruation. Also, menstrual bleeding is generally of longer duration than the normal period. The fact that sexual intercourse occurs in the luteal phase could cause anovulation and menstrual bleeding might last for a few days or even longer due to the high oestrogen content of the blood.
Advocacy actions:
Raise awareness among key groups, including women and girls, men, civil society, the media and health providers about the true purpose, effects, complications and alternatives to FGM
Raise awareness among key groups about the health risks of FGM and that any woman or girl experiencing sexual difficulties should seek medical consultation and referral, including when their genital area has been damaged by FGM
Promote the right to access the full range of sexual and reproductive health services, including birth control and abortion, without any restrictions on the grounds of social or religious beliefs, or lack of access to health services
Ensure that any woman or girl under the age of 18 who experiences reproductive health problems is able to access the services and can be referred to, and receive the interventions from, a doctor, midwife or other health worker of her choice
Support national and international efforts to end FGM, and to provide full protection to the rights of women and girls
Promote the human rights of women and girls who have been subjected to FGM and child brides to ensure their full and equal rights, including physical safety and full access to the same health services and programmes as other women and girls. 827ec27edc
